Responsible for tracing and monitoring contacts of people diagnosed with infectious diseases. Responsible for notifying individuals of their exposure and providing support to all isolated and/or quarantined individuals. Ensures the safety and prevention of spread of infectious diseases. Use of digital surveillance tools and assists with the coordination and expansion of staffing resources.



  • Responsible for case investigation for those who have been diagnosed with an infectious disease.
  • Identify and support close contacts who may have been exposed to the case (someone diagnosed with an infectious disease).
  • Daily monitoring of quarantined households through telephone, video conferencing, or other means of remote communication.
  • Use of digital surveillance monitoring tools to monitor for daily symptoms of infectious diseases.
  • Coordination of support services for quarantined or isolated households.
  • Participate in broad engagement to foster an understanding and acceptance of local case investigation and contact tracing.
  • Ensure services and/or other items requested are provided to quarantined or isolated households.
  • Attend ongoing trainings related to contact tracing, epidemiology, or public health activities that help to prevent the spread of infectious diseases.
  • Contributes to the overall quality of the unit’s service provision by developing and coordinating work teams and by reviewing, recommending and implementing improved policies and procedures.
  • Represents the Pueblo with dignity, integrity, and a spirit of cooperation in all relationships with staff and the public.

Education and Experience:

High School diploma or General Equivalency Diploma (GED): 4 years of experience in investigations, case management, admin work, or transferrable tasks.                                                  


Required Knowledge and Skills

Knowledge of:

  • Digital technology and use of remote connectivity tools.
  • Jemez Pueblo culture and Towa language, preferred.
  • Record management principles and practices.
  • Techniques for dealing with a variety of individuals from various socio-economic, ethnic and cultural backgrounds, in person and over the telephone.
  • Computer software utilized by the clinic and/or public health programs.


Skill in:

  • Adhere to all compliance and confidential guidelines including HIPAA Compliance and Privacy Act.
  • Providing customer services in the most cost effective and efficient manner.
  • Performing contract tracing procedures in an efficient and effective manner.
  • Preparing clear and concise reports, records of work and other written materials.
  • Using software applications related to duties.
  • Using tact, discretion and prudence in dealing with customers.
  • Using initiative and independent judgment within established procedural guidelines.
  • Organizing, prioritizing and coordinating work activities, coordinating multiple activities and meeting critical deadlines.
